What is physician-led weight management and how does it differ from dieting?

Physician-led weight management treats obesity as a chronic medical condition — not a willpower failure. A doctor assesses your metabolic health, identifies biological barriers to weight loss (such as elevated hunger hormones or insulin resistance), and creates an individualised plan that may include medication, nutrition guidance, and behavioural support. This is evidence-based medicine. Dieting alone addresses behaviour; physician-led care addresses the underlying biology. At Élan Clinic, every patient is assessed by a doctor before any treatment begins.

How much does weight loss treatment cost at Élan Clinic?

An initial 45-minute consultation with a physician costs €120. Ongoing monthly support packages start from €149/month. Medication costs are separate: semaglutide (Ozempic) runs approximately €88–95/month; Wegovy €140–200/month; tirzepatide (Mounjaro) €180–350/month — all at full price without insurance subsidy for weight loss indications. Estonian health insurance (Haigekassa) does not cover GLP-1 medications for obesity (only for type 2 diabetes). Video consultations are available, including for patients travelling from Finland.

Is semaglutide (Ozempic / Wegovy) safe for weight loss?

Under medical supervision, semaglutide has a well-established safety profile. The most common side effects are nausea, reduced appetite, and gastrointestinal discomfort — particularly in the first weeks of treatment when the dose is being increased gradually. In the STEP 1 clinical trial (Wilding et al., NEJM 2021; n=1,961), approximately 5% of participants discontinued due to adverse events. Semaglutide is contraindicated in pregnancy, during breastfeeding, and in individuals with a personal or family history of medullary thyroid carcinoma or multiple endocrine neoplasia. A physician assessment before starting is not optional — it is required. Élan Clinic evaluates every patient individually.

Does weight come back after stopping GLP-1 medication?

In most cases, yes — a significant portion of weight returns after stopping GLP-1 therapy. The STEP 1 extension study (Wilding et al., Diabetes, Obesity and Metabolism, 2022) followed patients who stopped semaglutide after 68 weeks: within 12 months of stopping, approximately two-thirds of the weight lost was regained. This is not a personal failure — obesity is a chronic condition, and removing the treatment removes its effect, just as stopping blood pressure medication would raise blood pressure again. What is the difference between Ozempic and Wegovy?

Both contain semaglutide — the same active ingredient — but they are approved for different indications and come in different maximum doses. Ozempic is approved for type 2 diabetes (up to 2 mg/week). Wegovy is approved specifically for obesity management (up to 2.4 mg/week — a higher dose). In the context of weight loss, Wegovy is the correctly indicated product. Some patients are prescribed Ozempic off-label for weight loss if Wegovy is unavailable or if they also have type 2 diabetes. The right choice depends on your diagnosis, availability, and your doctor's assessment.

What is the difference between semaglutide and tirzepatide (Mounjaro)?

Semaglutide (Ozempic, Wegovy) activates GLP-1 receptors — hormones that reduce appetite and slow gastric emptying. Tirzepatide (Mounjaro) activates both GLP-1 and GIP receptors simultaneously. In the SURMOUNT-1 trial (Jastreboff et al., NEJM 2022; n=2,539), tirzepatide achieved approximately 15–21% body weight reduction at the highest dose (15 mg) over 72 weeks, compared to approximately 12–15% with semaglutide in comparable trials. Mounjaro tends to produce greater weight loss for most patients, but the right choice depends on individual health profile, tolerability, and cost. Both require a prescription and medical oversight.

How long does it take to see results with GLP-1 weight loss therapy?

Most patients notice reduced appetite within the first 2–4 weeks. Visible weight loss typically begins within 4–8 weeks. Doses are increased gradually over several months — usually in 4-week intervals — to minimise side effects. The greatest cumulative weight loss is typically achieved between 12–18 months of treatment. Individual results vary considerably based on starting weight, dose reached, lifestyle factors, and adherence.

Do I need a prescription for Ozempic or Mounjaro in Estonia?

Yes. Ozempic, Wegovy, and Mounjaro are prescription-only medicines throughout the EU, including Estonia. A prescription from a licensed physician is required. At Élan Clinic, the initial consultation includes a full medical assessment. If you are an appropriate candidate, your physician will issue a prescription. The consultation can be conducted in-person in Tallinn or by video, including for Finnish or other international patients.

Can patients from Finland travel to Élan Clinic in Tallinn for weight loss treatment?

Yes. Élan Clinic is located at Sepapaja 12/1, Tallinn — approximately 2 hours by ferry from Helsinki. We offer consultations in English and Finnish, and a significant proportion of our patients travel from Finland. Your initial assessment can be conducted by video before you visit, and follow-up care can continue remotely. Who is a suitable candidate for GLP-1 weight loss treatment?

EU clinical guidelines generally indicate GLP-1 therapy for adults with a BMI ≥30 (obesity), or BMI ≥27 with at least one weight-related health condition such as type 2 diabetes, hypertension, or obstructive sleep apnoea. It is not appropriate for pregnant or breastfeeding women, or those with personal or family history of medullary thyroid carcinoma or multiple endocrine neoplasia type 2. Full individual assessment is required. At Élan Clinic, eligibility is determined by a physician — never by algorithm alone.

What results can I realistically expect from medical weight management?

Published clinical trial data: semaglutide 2.4 mg (Wegovy) produced an average 14.9% body weight reduction over 68 weeks vs placebo (Wilding et al., NEJM 2021). Tirzepatide 15 mg (Mounjaro) produced approximately 20.9% reduction over 72 weeks vs placebo (Jastreboff et al., NEJM 2022). These are group averages — individual results vary. Results are best maintained when medication is combined with dietary change and physical activity. We do not quote outlier results. We tell patients what the evidence actually shows.

Are there weight loss medication options other than Ozempic or Mounjaro?

Yes. Other evidence-based options available in the EU include: liraglutide (Saxenda, daily injection, approximately €80–120/month — lower efficacy than semaglutide), naltrexone/bupropion (Mysimba, oral tablet, approximately €40–70/month), and orlistat (Xenical, oral, approximately €20–40/month — lowest efficacy but low cost). These are appropriate for some patients depending on health profile, cost constraints, and preferences. At Élan Clinic we discuss all options — we are not a single-medication clinic.

Is GLP-1 weight loss treatment suitable for women after menopause?

Yes, and it is often particularly relevant. Menopause is associated with hormonal shifts that promote fat accumulation — especially visceral (abdominal) fat — independent of caloric intake. GLP-1 receptor agonists (semaglutide, tirzepatide) act on appetite and metabolic regulation through mechanisms that are not dependent on oestrogen levels. Clinical trials included substantial numbers of post-menopausal women, and efficacy data support their use in this group. As with all patients, individual assessment and ongoing medical supervision are required.
